Visa Application Checklists & Fees

The Citizenship Department for Home Affairs provides information regarding visas, permits and residency applications.

The following applications are available to download:

• Visa 
• Application for temporary residence permit
• Application for permanent residence permit

Immigration Health Check Forms

You will need to have a health check for immigration purposes. To organise a health check, you will need to complete the health check forms and make an appointment with an approved doctor.

Medical Report (BI-811)
Radiological Report (BI-806)

Immigration Police Check Forms

If you want to immigrate to South Africa or as a temporary foreign worker you and your dependants may have to provide police certificates when you submit your application. This is usually for applicants 21 years or older from all countries where the person resided one year or longer.
