Immigration Health Check Forms
You and any accompanying family will need to have a health check for immigration purposes depending how long you intend to stay in New Zealand. To organise a health check, you will need to complete the health check forms and make an appointment with an approved doctor.
The check will include a medical and radiological examination and tests for HIV/AIDS, Hepatitis B and Hepatitis C. You will also be advised what checks are required for family members included in their visa application.
If you are staying less than 6 months you do not need to provide a medical certificate. If you are staying between 6-12 months you must provide a completed Temporary
X-Ray Certificate. If you are staying more than a year you must provide a completed
Medical and Chest X-Ray certificate.
In some countries, Immigration New Zealand selects
medical panels of registered medical practitioners and/or radiologists. You may have to pay the fee for the medical practitioner and/or radiologist to complete their examinations and tests, and for any further medical examinations we may require.
Immigration Police Check Forms
Immigration New Zealand requires a statement from the police authority, of each country that an overseas trained doctor has lived in for more than 12 months over the past 10 years. If your work permit is for 2 or more years you must provide police certificates from any country you have lived in for five years or more since you turned 17.
